It's exactly like "How have you been?". We use "어떻게 지내셨어요?" How have you been? "어떻게 지내고 계세요?" How are you doing? <So, both of them are basically same.> "어떻게 지냈어요?" How have you been? "어떻게 지내고 있어요?"
